Service Description

Please complete the intake forms and the Terrain Ten Survey for this consultation. Forms and a link to the survey will be provided by email. Please return no later than 48 hours before your scheduled consultation. Failure to provide the necessary documents within a timely manner may result in your consultation being rescheduled.

Cancellation Policy

Please reschedule within 48 hours of your appointment time. Clients may request a full refund within 24 hours of booking a consultation or package. Clients who successfully on-board to Cronometer Pro, MyMojoHealth, and/or complete their first session of a package are not eligible for a full refund. Package extensions will be determined on a case by case basis and must be requested in writing before the package expires.
