Service Description

Please complete the intake forms and the Terrain Ten Survey for this consultation. Forms and a link to the survey will be provided by email. Please return no later than 48 hours before your scheduled consultation. Late paperwork will cause the consultation to be rescheduled.

Cancellation Policy

Please reschedule within 48 hours of your appointment time. Clients may request a full refund within 24 hours of booking a consultation or package. Clients who successfully on-board to Cronometer Pro, MyMojoHealth, and/or complete their first session of a package are not eligible for a full refund. Package extensions will be determined on a case by case basis and must be requested in writing before the package expires.
